Zippin, MD, PhD; Artur Zembowicz, MD, PhD<\/p>\n<p><!--more--><\/p>\n<p><strong>Context<\/strong>.\u2014Soluble adenylyl cyclase (sAC) is an enzyme\u00a0that generates cyclic adenosine monophosphate, a signaling\u00a0molecule involved in regulating melanocyte functions.<\/p>\n<p>R21, a mouse monoclonal antibody against sAC, shows a\u00a0striking pan-nuclear staining in lentigo maligna, indicating\u00a0possible utility for diagnosis and margin assessment.<\/p>\n<p><strong>Objective<\/strong>.\u2014To evaluate R21 in the diagnosis and\u00a0evaluation of margins in lentigo maligna.<\/p>\n<p><strong>Design<\/strong>.\u201431 re-excision specimens for lentigo maligna\u00a0were evaluated for R21 expression using previously\u00a0published protocol. In addition, 123 cases including 41\u00a0lentigo malignas, 30 non\u2013lentigo maligna-type melanomas,\u00a038 lentigos, and 44 nevi were evaluated using a modified\u00a0stringent protocol to eliminate all nonmelanocyte staining.<\/p>\n<p><strong>Results<\/strong>.\u2014The sensitivity of nuclear staining with R21 in\u00a0lentigo maligna was 87.8%. Nuclear expression of sAC was\u00a0observed in 40% of other melanomas and 2.3% of benign\u00a0nevi. R21 did not stain nuclei of resting melanocytes but\u00a0was observed in 28.9% of melanocytic hyperplasias. These\u00a0cases were easily distinguished from lentigo maligna in\u00a0routine sections. R21 staining facilitated extent of the\u00a0lesion in resection margins. In cases examined under the\u00a0less stringent conditions, interpretation was facilitated by\u00a0comparing R21 and Mart1\/Melan A staining. Greater than\u00a09 pan-nuclear staining melanocytes within one high-power\u00a0field along with a pan-nuclear sAC\/melan A ratio greater\u00a0than 0.5 was consistent with a positive margin whereas 5\u00a0or less pan-nuclear staining melanocytes along with a sAC\/\u00a0melan A ratio of less than 0.3 constituted a negative\u00a0margin.<\/p>\n<p><strong>Conclusion<\/strong>.\u2014R21 is a useful diagnostic adjunct in the\u00a0diagnosis and evaluation of margins in re-excision specimens\u00a0in lentigo maligna.<\/p>\n<p>(Arch Pathol Lab Med.\u00a02012;136:1\u20137; doi: 10.5858\/arpa.2011-0617-OA)<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Archives of Pathology &amp; Laboratori Medicine.\u00a0Expression of Soluble Adenylyl Cyclase in Lentigo MalignaCynthia M.
