Pinacoteca de Sao Paulo, Brasil.<\/em><\/span><\/p>\n<p align=\"JUSTIFY\">Elie Metchnikoff \u2013 Microbi\u00f3logo 1845 -1916, descubri\u00f3 los fermentos l\u00e1cticos como terapia para el s\u00edndrome de fermentaci\u00f3n intestinal.<\/p>\n<p align=\"JUSTIFY\">En los \u00faltimos a\u00f1os se ha postulado que la flora intestinal anormal (disbi\u00f3tica) podr\u00eda ocasionar patolog\u00edas fuera del intestino, por ejemplo, en la piel (acn\u00e9, ros\u00e1cea, psoriasis, dermatitis at\u00f3pica etc.) Con respecto al acn\u00e9, desde hace un par de a\u00f1os se ha estado propagando la posibilidad de intentar controlar la microbiota intestinal y de la cara utilizando productos farmac\u00e9uticos que contengan bacterias y que puedan regular la flora del intestino y de la piel sin necesidad de recurrir a los antibi\u00f3ticos. Esa posibilidad es a todas luces atractiva e invita a comenzar a utilizarla como una terapia mas hol\u00edstica y ecol\u00f3gica. Sin embargo, eso no puede decidirse alegremente sin comprobaci\u00f3n. Paralelamente, desde hace unos meses hemos presenciado como reconocidos investigadores han expresado vehementemente que el \u201c<em>tiempo de uso de los antibi\u00f3ticos en el acn\u00e9 ha llegado a su fin<\/em>\u201d e inclusive, hace unos d\u00edas asist\u00ed a una conferencia de un popular orador quien expresaba que \u201c<em>el galeno que use antibi\u00f3ticos en el manejo del acn\u00e9 y ros\u00e1cea est\u00e1 totalmente desactualizado<\/em>\u201d, todo ello lo manifest\u00f3 haciendo una morisqueta facial descalificativa a todos los que, como yo, utilizamos antibi\u00f3ticos en el manejo del acn\u00e9 y\/o ros\u00e1cea.<\/p>\n<p align=\"JUSTIFY\">Esa postura espole\u00f3 mi intelecto y me dediqu\u00e9 a realizar una b\u00fasqueda bibliogr\u00e1fica sobre el tema que arroj\u00f3 conclusiones mas significativas y sujetas a la necesaria investigaci\u00f3n sobre el dilema planteado. The purpose of this study was to test the efficacy of a dietary supplement containing probiotics (Bifidobacterium breve BR03 DSM 16604, Lacticaseibacillus casei LC03 DSM 27537, and Ligilactobacillus salivarius LS03 DSM 22776) and botanical extract (lupeol from Solanum melongena L. and Echinacea extract) in subjects with mild to moderate acne over an 8-week study period.<\/span><\/span><\/span><\/span><\/span><\/span><\/p>\n<p><span style=\"font-family: Times New Roman, serif;\"><span style=\"font-size: medium;\"><b><span style=\"color: #212121;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: small;\"><span lang=\"en-US\">Methods:\u00a0<\/span><\/span><\/span><\/span><\/b><span style=\"color: #212121;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: small;\"><span lang=\"en-US\">Monocentric, randomized, double-blind, four-arm, placebo-controlled clinical study involving 114 subjects.<\/span><\/span><\/span><\/span><\/span><\/span><\/p>\n<p><span style=\"font-family: Times New Roman, serif;\"><span style=\"font-size: medium;\"><b><span style=\"color: #212121;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: small;\"><span lang=\"en-US\">Results:\u00a0<\/span><\/span><\/span><\/span><\/b><span style=\"color: #212121;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: small;\"><span lang=\"en-US\">A significant (p &lt; 0.05) effect on the number of superficial inflammatory lesions was reported over the study period in the subjects taking the study agent (group II) (-56.67%), the botanical extracts (group III) (-40.00%), and the probiotics (group IV) (-38.89%) versus placebo (-10.00%). A significant (p &lt; 0.05) decrease in mean desquamation score, sebum secretion rate, and porphyrin mean count versus baseline was also reported, and the effect was most evident for group II. The analysis of log relative abundance after 4 and 8 weeks of treatment compared with baseline showed a significant (p &lt; 0.01) decrease in Cutibacterium acnes and S. aureus, along with a contextually and significant (p &lt; 0.05) increase in Staphylococcus epidermidis, especially in group II. No significant changes were reported for group I.<\/span><\/span><\/span><\/span><\/span><\/span><\/p>\n<p><span style=\"font-family: Times New Roman, serif;\"><span style=\"font-size: Because both the gut-skin axis and dysregulation of insulin signalling have been implicated in the pathogenesis of adult acne, we designed the current study to evaluate the effect of supplementation with the probiotic strain Lactobacillus rhamnosus SP1 (LSP1) on skin expression of genes involved in insulin signalling and acne improvement in adult subjects. A pilot, randomised, double-blinded, placebo-controlled study was conducted with 20 adult subjects (14 females and 6 males; mean age: 33.7\u00b13.3 years) with acne. Over a 12-week period, the probiotic group (n=10) consumed a liquid supplement containing LSP1 at a dose of 3\u00d7109 cfu\/day (75 mg\/day), whereas the placebo group (n=10) received a liquid lacking probiotics. Paired skin biopsies &#8211; one obtained before treatment initiation and one obtained at the end of the 12-week treatment period &#8211; were analysed for insulin-like growth factor 1 (IGF1) and forkhead box protein O1 (FOXO1) gene expression. The clinical criterion for efficacy was the investigator&#8217;s global improvement rating on a five-point scale. Compared with baseline, the probiotic group showed a 32% (P&lt;0.001) reduction, as well as a 65% increase (P&lt;0.001) in IGF1 and FOXO1 gene expression in the skin, respectively. No such differences were observed in the placebo group. Patients in the probiotic group had an adjusted odds ratio of 28.4 (95% confidence interval = 2.2-411.1, P&lt;0.05) to be rated by physicians as improved\/markedly improved (versus worsened or unchanged) compared with the placebo group.
