Aprobado por la FDA Ipilimumab para el Melanoma Metastásico

El 25 de marzo de 2011, la FDA anunció la aprobación del medicamento IPILIMUMAB para el tratamiento del melanoma metastásico, les anexo el link del anuncio y el documento en pdf con la información que llevará el prospecto.

    Si bien es un medicamento «prometedor» el tiempo dirá y esperemos por el bien de nuestros pacientes que asi sea. Los efectos adversos mediados por el sistema inmune pueden ser fatales, tanto asi que la FDA lo aprueba el 25 de marzo y emite esta nota de alerta hace apenas 4 dias (6/4/11) sobre el balance riesgo-beneficio y como prevenir o detectar a tiempo las reacciones fatales por ipililumab

    Yervoy (ipilimumab): Risk Evaluation and Mitigation Strategy (REMS) – Severe Immune-Mediated Adverse Reactions

    AUDIENCE: Hematology-Oncology

    ISSUE: Bristol-Myers Squibb informed healthcare professionals about the risk evaluation and mitigation strategy (REMS), developed in collaboration with FDA, that is required to ensure that the benefits of Yervoy outweigh the risks of severe and fatal immune-mediated adverse reactions. The Yervoy REMS consists of a Communication Plan to inform healthcare professionals of the serious risks of Yervoy, to facilitate early identification of these risks, and an overview of recommended management of patients with moderate or more severe immune-mediated adverse reactions.

    BACKGROUND: Yervoy was approved March 2011 with the Prescribing Information including a Boxed Warning stating that use of the product can result in severe and fatal immune-mediated adverse reactions due to T-cell activation and proliferation. These immune-mediated reactions may involve any organ system; however, the most common severe immune-mediated adverse reactions are enterocolitis, hepatitis, dermatitis (including toxic epidermal necrolysis), neuropathy, and endocrinopathy. The majority of these immune-mediated reactions initially manifested during treatment; however, a minority occurred weeks to months after discontinuation of Yervoy.

    RECOMMENDATION: Healthcare providers were advised to read the boxed warning and accompanying full Prescribing Information for a complete description of these risks and their management and were advised to discuss the risks that may be associated with therapy with patients and their caregivers. Clinicians were advised to:

    permanently discontinue Yervoy and initiate systemic high dose corticosteroid therapy for indentified severe immune-mediated reactions.
    assess patients for signs and symptoms of enterocolitis, dermatitis, neuropathy and endocrinopathy and evaluate clinical chemistries including liver function tests and thyroid function tests at baseline and before each dose.

    Esto es verdad Dr.,por eso la FDA aprobo el uso de ipilimumab(MX-10)a dosis de 3,5 mg/Kg,los anticuerpo monoclonales no son inocuos y deben ser manejados por un personal capacidado,la aprobacion de ipilimumab es una realidad con mas de 15 anos estudios.

No me entendieron los colegas, «tremendo logro» es un sarcasmo; qué logro se puede obtener con una droga que prolonga la vida por dos meses y ofrece efectos colaterales próximos a la muerte y un colsto elevadísimo.     Como conozco a José Rafael Sardi de muchos años, sabía que su intervención algo encerraba, por lo menos el ecomillado de su frase «tremendo logro», seguro que contenía ironía o sarcasmo. Cuando me disponía a escribir algo sobre el tema, apareció su aclaratoria. Comparto con él, el principio de que el único tratamiento realmente curativo para el melanoma maligno es la extirpación quirúrgica de la lesión en sus etapas más precoces, con excelentes expectativas de supervivencia a los 10 años, si la lesión se encuentra «in situ». (100% según los grupos cooperativos estudiosos de MM, como lo confirma la última guía para su manejo publicada en el British Journal of Dermatology.(Marsden JR, Newton-Bishop JA, Burrows L, et al.Revised U.K Guidelines for the Management of Cutaneous Melanoma. British Journal of Dematology 2010;163(2):238-256). Mi forma de pensar es que la disminución de las cifras de muerte por MM realmente disminuirían con una campaña intensa de educación, advirtiendo a toda la población sobre los riesgos de la lesión, como lo asumió ya hace muchos años EEUU, Inglaterra y Australia. El control periódico, especialmente en pieles suceptibles tipo I y II de Fitzpatrick, sería lo más efectivo.

    Con eso no quiero decir que me opongo a todas la investigaciones biológicas que actualmente se efectúan, en la búsqueda de tan deseada solución para el MM metastásico. Hasta el momento actual no hay cura para el MM metastásico y realmente los ensayos con el ipilimumab(MX-10), están muy lejos de alcanzar la meta deseada, además con alto riesgo de tormenta citocínica e inclusive muerte y a un costo exorbitante. Valdrá la pena someter a un paciente a los inmensos riesgos de todo tipo como lo señala el Dr. Juan Antonio Chassaigne en su comentario y que pude comprobar leyendo el artículo original en el NEJM ?. Creo que desde el punto de vista filosófico, habrá que colocar estas situaciones críticas de un paciente con MM metastásico, en la balanza de la ecuanimidad, buen razonamiento y sereno criterio terapéutico, analizando todos los pro y los contras, antes de someterlo a este tipo de terapia.

    Hola a Todos, la finalda de nosotros (Wilmer, Juan Antonio y mia) solo fue informar a nustra comunidad dermatologica latinoamericana de la aprobacion por la FDA del ipilimumab (Yervoy).
    En ningun momento se ha querido decir que este este farmaco sea la cura para el Melanoma Maligno, de hecho su uso fue aprobado para tratamiento de Melanoma Metastasico, siguiendo una evaluacion de ESTRATEGIA DE MITIGAGACION de RIESGOS.

    La Prevencion y diagnostico temprano es la mejor herramienta con la que contamos, de hecho yo tengo dando charlas de Fotoproteccion en la Infancia desde el año 2002- asistiendo a colegios en la ciudad de Valencia para educar y orientar a los niños y jovenes con respecto al daño que produce la exposicion solar. Desde hace algun tiempo me acompaña en este esfuerzo mi colega Naty Rondon , en Caracas.
    Yo me puse en contacto con la EPA(Agencia de Proteccin Ambiental de Estados Unidos) que tiene el Programa SunWise de educación medioambiental y sanitaria con el objetivo de enseñar al público a protegerse correctamente de la sobreexposición al sol mediante la realización de actividades en el aula, en la escuela y en la comunidad y ellos me enviaron su protocolo de trabajo para las escuelas hispanas.

    Ya tendremos tiempo para ver, evaluar y estudiar que ocurre con el uso del Yervoy, en USA y los otros paises que vayan aprobando su uso.

    1. FOTOEDUCACION.
    2. AUTOEXAMEN DE PIEL PERIODICAMENTE
    3. Evaluacion medica especializada.

    En una discusión reciente en una Clínica de Tumores, al hablar de las terapias del MM, se hizo la observación de que estas terapias inalcanzables para el bosillo latinoamericano, tienen un aspecto instructivo de Educación Continuada… ellas nos hacen caer en cuenta de cuales son los desarrollos de la investigación molecular del Mmporque todas se basan en eso… en los últimos 50 años las terapias diferentes a la Cirugía, se originan de allí… desde el BCG,hasta la Dacarbazina, el Proleuquín, el interferón y las cada vez más caras… ha sido tan grande el conocimiento de esto, que al MM hoy no sólo se le clasifica por los conocidos patrones clínicos e histológicos, sino también, por sus patrones moleculares.

  14. Claro que sí Dr. Ramos, mi comentario es irónico. Lamentablemente hoy por hoy, un melanoma maligno metastásico no tiene un tratamiento efectivo. Sólo estar pendiente de la aparición de las metástasis, y si se pueden erradicar quirúrgicamente hacerlo rápido. No es bueno hablar en primera persona, pero el manejo de los melanomas malignos en nuestro servicio por unos cuantos años me permite hablar como lo hago; yo conozco las estadisticas y las acepto, pero no siempre las comparto. El hecho que un determinado tratamiento pueda alargar la vida de un enfermo por dos meses no dice absolutamente nada, no es un logro en el manejo del melanoma maligno metastásico. Desgraciadamente las metástasis del melanoma maligno pueden acabar con la vida en unos días como en años. Yo uso mucho un pensamiento del Dr. B. Ackerman que refleja cabalmente el tema que discutimos: «El melanoma cutáneo primario, que todavía no ha metastizado es curable por medio de la escisión in toto; ningún tratamiento adicional es requerido. Una vez que la metástasis se hace aparente clínicamente, es probable que sí el paciente vive lo suficiente y no muera de otra causa, la muerte, con el tiempo, ocurra por lo efectos de este. Si va a ocurrir en meses o décadas, no puede ser determinado.»
    Desde luego, todos queremos que se descubra un droga que nos ayude a salvar los enfermos, pero, hasta ahora los «tiros» no van por ahí. También sé que Uds. estan informándonos en este «blog», no es que están proponiendo que usemos una droga que apenas sale a la luz pública. De acuerdo?

    Desde mi punto de vista, la educación continuada en melanoma o cualquier otra disciplina dermatológica -lo que creo que es intención de blogs como Piel-l- es mantener actualizados para sus usuarios, los conocimientos sobre un tema… como no todo el mundo dermatológico se actualiza por su esfuerzo personal, por twitter -todavía no… aunque sé que algunos ya empiezan, incluído yo…- y similares, estos blogs en Internet seguirán con su vigencia -si no se transforman- por algunos años… mantengamos en la cabeza la idea de que las cosas, evolucionan… se desarrollan… eso de ver actualmente a los jóvenes tecleando y abstraidos de el entorno, tiene su explicación…. no sólo están chateando… fundamentalmente, se están informando… un «trino» de 40 palabras máximo, se ajusta a la filosofía de que «lo bueno si breve, 2 veces bueno»… no nos asustemos … más bien entendámoslo… para allá marcha la informática en el mundo actual y eso abarca a la dermatología… yo agradezco la información sobre ipilimumab y reconozco la buena intención de los autores de ella, más como interesado primordialmente en puntualizar, en recalcar, que las cosas deben tener un orden jerárquico de explicación y conocimiento, con lo del ipílimummab -pienso yo- la cogimos por la cola… es decir en el conocimiento del MM y su divulgación., ipilimumab, es una consecuencia del conocimiento de muchas cosas que han sucedido antes… pero están aún mal conocidas y nuestra obligación primera, para el provecho de todos a la par, es recalcarlas…por ejemplo… que és el BRAF? que son las kinasas?… Cree Piel-L que todos sus usuarios lo tienen claro?… eso -jerarquicamente en el MM- está mucho antes del ipilimumab… caer en cuenta de que «coger al toro por los cachos», es sano… y hacer educación continuada con ese logo es admirable… lo que digo no es sorna, ni es «la puterie»… es una colaboración con la mejor intención y con todo respeto.

    Por si de algo sirve, quiero dejar patente mi jerarquia en las cosas de la dermatología 1-la definición y la clínica 2- la patogenia, 3-la histología y 4-lo molecular… la terapia aunque tiene mucho de ciencia, tiene más de arte…. pregúntale a tu colega de al lado como pone una inyección con o sin dolor y te darás cuenta de la importancia de tener la seguridad de que como tú lo haces, lo haces bien…

    es lógico que si me hé hecho entender bien -defecto mío- para mí tampoco ipilimumab es «tremendo logro»… es un paso para conocer y nada más; en nuestras latitudes… tuve más de un paciente a quienes después de extirpar su lesión primaria, alguien les hizo vender la casa y la finca, para ponerles interferón o proleukin y terminar «muy llenos de vida»… en el crematorio… no sé si un programa de la AAD que calcula las sobrevida del paciente con MM de acuerdo a edad, localización, breslow, adenopatías, sigue vigente, porque lo encontré útil en su momento… al menos para sin eufemismos ni engaños orientar a los pacientes… puede haber cambiado… recordemos… este tema, en dermatología. es quizás el más controversial…
